What we look at

What we look at for GPs

We’re accountants, not coders. Clinical and billing decisions stay with you. We read your billing data and agreement so they’re compliant first and complete second.

Items 965 and 967: plan, then review

Since 1 July 2025, item 965 prepares a GP chronic condition management plan and 967 reviews it, up to every three months where clinically relevant. For patients registered in MyMedicare, bill through their registered practice. Plans made before 1 July 2025 stay valid until 30 June 2027, so recall those patients gradually.

Video items 92029/92030; practice nurse 10997, up to five a year.

BBPIP: is your 12.5% reaching you?

The Bulk Billing Practice Incentive Program pays 12.5% of eligible bulk-billed MBS benefits each quarter, split 50/50 between GP and practice. Your half is paid to the account you nominate in HPOS, for each site. If your banking details are still missing after four payment quarters, that payment is forfeited. Check that your agreement doesn’t count your share as practice income.

How much12.5% of the MBS benefits paid for eligible bulk-billed services, assessed quarterly and split 50/50 between the GP and the practice.
Who qualifiesThe practice registers through the Organisation Register, is registered in MyMedicare and bulk bills every eligible MBS service for all Medicare-eligible patients. Every eligible GP at that location must be linked to it.
How you are paidYour half is paid directly to you, into the bank account you nominate in your own HPOS account, with a separate nomination for each site you work at.
The trapIf your banking details are still missing after four payment quarters, that period’s payment is forfeited.
Trade-offJoining means bulk billing every eligible service. For a GP with a strong private billing base, the maths can go either way, so model it before your practice decides.
The patterns the regulator watches

The Professional Services Review looks at patterns: 80 or more attendances on 20 or more days in 12 months (the 80/20 rule), or 30 or more phone services on 20 or more days. We turn your monthly billing report into a one-page dashboard so you see the trend first.

Service fees and payroll tax, by state

A 65/35 or 70/30 split is a service fee. In NSW, the Bulk Billing Support Initiative offers a payroll tax rebate to clinics bulk billing at least 80% of GP services (70% outside metropolitan Sydney). Queensland and Victoria have their own rules.

Tax and wealth

Tax and wealth for GPs

The right structure

Most contractor GPs earn personal services income. We only recommend a company or trust when the rules and the numbers support it.

No surprise tax bills

PAYG instalments set to your real income, and a separate tax account.

Deductions done properly

Indemnity, AHPRA, RACGP or ACRRM fees, CPD and equipment, with records that stand up.

Super and insurance

Concessional contributions, carry-forward caps, Division 293 above $250,000, and income protection that fits how GPs earn →
